If autoimmune disease is the only reason, and if the bonding between the couple is strong, they could have opted for surrogacy. Now a days, with the advent of technology, you can grow multiple embroyos and do genetic testing to know if it is low risk or high risk. 
 

use the help of a surrogate and get a child or two. 
 

this case is clearly incomparably and breach of trust between couple.

Not at all an issue of health. There is definitely some fragment of "cheating" that happened. Don't have more info. Thelisina some friend's friend said. Not even sure if he's telling the truth either.
