*2. Corona Positive Testing Report - Xerox copy*.
*3. Original Hospital Admit Card with In Patient (IP) Number*.
*4. Doctor Prescription with Stamp & Sign of the Doctor*.
*5. Carry Original Aadhar Card & Xerox copy of the person, who is going to Hetero office to collect Remdesivir Injection. Show them Original Aadhar Card and take it back. Submit Colour Xerox copy*.
*6.No Cards are accepted*.

3 . ఒరిజినల్ హోస్ప్పిటల్ అడ్మిట్ కార్డు .దానిపై పేషేంట్ యొక్కఇన్ పేషెంట్ నెంబర్ తప్పకుండ ఉండాలి .
4 . డాక్టర్ ప్రిక్రిప్షన్ తప్పకుండ ఉండాలి దానిపై డాక్టర్ సంతకం మరియు స్టాంప్ తప్పకుండ ఉండాలి.
5 . ఎవరైతే Remdesivir Injection తీసుకు రావడానికి వెళ్తారో , వారు వారియొక్క ఒరిజినల్ ఆధార్ కార్డు తో పాటు జిరాక్స్ కాపీ ని కూడా తీసుకుని వెళ్ళాలి. ఒరిజినల్ ఆధార్ కార్డు చూపించి కలర్ జిరాక్స్ కాపీ వారికి ఇవ్వాలి.
